According to University Policy, George Mason University (GMU) requires all vendors fill out the Taxpayer Identification and Proposal of Ser
vice Application forms the first time when the university purchases any services.Please download the forms to fill out and fax them back to us
at the fax number below.
Furthermore, all Vendors are also required to register with eVA, the Commonwealth of Virginia's electronic procurement system at http://www.eva.state.va.us/, before the university can process vendor invoices.
